Families In Transition works closely with families with children throughout Santa Cruz County who are homeless or at risk of homelessness. A variety of housing strategies are provided to support families in securing safe and stable homes for their families. A range of opportunities are available depending upon family situation.
Services include one-on-one case management where families and their case manager work together to identify barriers to self-sufficiency and develop an individualized housing stability plan so that families can regain their self-reliance. Forms of direct assistance may include: One time rental assistance payments to prevent eviction or obtain housing, time limited phased rental assistance, housing scholarships while a parent is completing a job training program, transitional housing for up to 18 months, and shared supportive housing for parents in recovery.
The County of Santa Cruz has multiple Senior Social Workers out-stationed at Families in Transition in order to provide additional services and support to our program participants.
All programs and services focus on the development of an individual plan of action to promote financial independence, healthy child development, and increased family well-being.
