Families In Transition is now participating in coordinated entry, a community wide effort to assess families and individuals to determine which participating agency would best be able to assist them.

FIT HOME Tenant-Based Rental Assistance and housing stability support, What is it?
Case Management, tenancy education, budgeting, financial literacy, resource referral which supports housing retention (employment, utility assistance, etc..), peer counseling, assistance accessing/maintaining mainstream benefits, and individual housing stability plan. Clients pay 30% of their income towards rent, and HOME funding subsidizes the balance.
Eligibility
Applicants must have household incomes at or below 60% of the County’s area median income (AMI), adjusted by household size, as published by the California Department of Housing and Community Development (HCD). All applicants must certify that they meet the household income eligibility requirements for the Program and have their household income documented. The income limits in place at the time the application for assistance is submitted will apply when determining the applicant's income eligibility.
Referrals
FIT does not accept program specific community based referrals for the HOME program. Enrollment in the HOME program is made by internal referrals of eligible households in order to ensure that these households receive the tailored assistance appropriate to their individual situation.
